In this project the performance and implementation of interactive web-based programs for telemonitoring and e-consultation will be examined, with particular emphasis on self-care management of patients with chronic complaints. The research question is: to what extent monitoring and e-consultation encourage self-care and high quality of care? The aim of this project is to determine design and evaluation criteria for self-care management via web-based programs in Primary Care. Several features of the web-based programs will be evaluated: E-surveillance (glycaemic values, dietary values registration and data exchange among care providers); E-monitoring (data transmission process; feedback, instruction and education about self-care management); E-triage (primary evaluation of a complaint ); E-care (treatment at distance through e-consultation via secure systems); E-learning (online education regarding self-care management.
